Report of Technical Details of a Facility (Form S1)

Note :
Form S1 should be submitted at the time of permit application if the facility produces or anticipates to produce Schedule 1 chemicals, or submitted to the Director-General of Trade and Industry in advance whenever there are changes to the information related to a Schedule 1 facility.

Annual Reports on Schedule 1 Chemicals -

1. Past Activities at the Facility during the Previous Year (Form S1-1) 
   
   
2. Transfer / Import / Export of Each Chemical during the Previous Year (Form S1-2) 
   
   
3. Anticipated Activities at the Facility for the Coming Year (Form S1-3)
