the name of the facility, the owner, the company or enterprise operating it; |

the precise location of the facility including the address; |
| c. |
the major products manufactured at the facility; |
| d. |
the name and amount of each UDOC produced at the facility; |
| e. |
the main activities (e.g. production, processing, consumption, storage, re-packaging, distribution or research and development etc.) of the facility; |
| f. |
the number of plant and the name of each plant at the facility that produced UDOCs including PSF-chemicals; |
| g. |
the number of plant and the name of each plant at the facility that produced more than 30 tonnes of a PSF-chemical; and |
| h. |
the name and quantity of each PSF-chemical produced in each plant as stated in (g) above. |

| Such records shall be kept for not less that 3 years after the particular year. |
